The following names can be used on templates used to produce T4A Statement of Pension, Retirement, Annuity and Other Income Slips.

| RECIPIENTLASTNAME | Prints the recipient's last name as specified on the T4A slip: Last name (in capital letters), first name, and initial. |
| RECIPIENTFIRSTNAME | Prints the recipient's first name as specified on the T4A slip: Last name (in capital letters), first name, and initial. |
| RECIPIENTINITIALS | Prints the recipient's initials as specified on the T4A slip: Last name (in capital letters), first name, and initial. |
| RECIPIENTADDRESS | Prints the recipient's address. |
| BOX012 | Prints the recipient's social insurance number. |
| BOX013 | Prints the recipient's account number. |
| BOX016 | Prints the recipient's pension or superannuation. |
| BOX018 | Prints the recipient's lump-sum payments. |
| BOX020 | Prints the recipient's self-employed commissions. |
| BOX022 | Prints the recipient's income tax deducted. |
