| BOX10 | Prints the recipient's actual amount of dividends. |
| BOX11 | Prints the recipient's taxable amount of dividends. |
| BOX12 | Prints the recipient's dividend tax credit for dividends. |
| BOX13 | Prints the recipient's interest from Canadian sources. |
| BOX18 | Prints the recipient's Capital gains dividends. |
| BOX21 | Prints the Report Code. |
| BOX22 | Prints the recipient's identification number. |
