The following names can be used on templates used to produce T5018 Statement of Contract Payments Slips.

| RECIPIENTNAME | Prints the recipient's business name as specified on the T5018 slip. |
| RECIPIENTADDRESS | Prints the recipient's address. |
| RIN | (Box 24) prints the recipient's identification number, account number or SIN. |
| BOX12 | Prints the recipient's social insurance number. |
| BOX20 | Prints the period ending the T5018 information is for. |
| BOX22 | Prints the recipient's construction subcontractor payments. |
| BOX54 | Prints the payer's account number. |
