The following names can be used on templates used to produce T4 Slips.

| VOID | Prints an X when the T4 slip will be left blank. A T4 is left blank in cases where the number of T4s to be printed results in an unused T4 on a page. |
| YEAR | Prints the year the T4 information is for. |
| EMPLOYERNAME | Prints the employer's name as entered on the Print T4s settings window. |
| EMPLOYERADDRESS1 to 4 | Prints the employer's address as entered on the four lines on the Print T4s settings window. |
| EMPLOYEEADDRESS | Prints the employee's address. |
| EMPLOYEENAME | Prints the employee's name as specified on the T4 slip: Last name (in capital letters), first name, and initial. |
| BOX10 | Prints the two-letter code for the employee's province of employment. |
| BOX12 | Prints the employee's Social Insurance Number. |
| BOX14 | Prints the employee's total earnings. Note that this amount includes many of the amounts printed in the other boxes at the bottom of the form (OTHERBOX1AMOUNT - OTHERBOX6AMOUNT). The other items included in box 14 are: 30, 31, 32, 33, 34, 36, 38, 40, 42, 70, 71, 72, 73, 74, 75, 76, 77, 78, 79, & 80. |
| BOX16 | Prints the employee's C.P.P. deduction total. |
| BOX17 | Prints the employee's Q.P.P. deduction total. |
| BOX18 | Prints the employee's E.I. deduction total. |
| BOX20 | Prints the employee's R.P.P. deduction total. |
| BOX22 | Prints the employee's income tax deduction total. |
| BOX24 | Prints the employee's E.I. insurable earnings total. Note: This box is left blank if the amount is the same as the earnings in box 14. |
| BOX26 | Prints the employee's CPP/QPP pensionable earnings total. |
| BOX28CPP | Prints an X if the employee is exempt from C.P.P./Q.P.P. |
| BOX28EI | Prints an X if the employee is exempt from EI (Employment Insurance.) |
| BOX29 | Prints the employee's employment code. |
| BOX44 | Prints the employee's union dues total. |
| BOX46 | Prints the employee's charitable donations total. |
| BOX50 | Prints the employer's pension plan registration number as entered on the T4 Print Settings window. |
| BOX52 | Prints the pension adjustment amount as entered for the employee on the T4 List window. |
| BOX54 | Prints the employer's business number as entered on the Print T4s settings window. Note: The business number is not printed when the Copies per employee is set to 2. It is assumed that when you are printing two copies that you will be giving the form with the two copies to the employee, and the business number is not supposed to be printed on the employee's copies. |
| OTHERBOX1 - 6 | Prints the box numbers for "other" amounts (such as non-cash taxable benefits). |
| OTHERBOX1AMOUNT - OTHERBOX6AMOUNT | Prints the amounts of "other" amounts (such as non-cash taxable benefits). |
